What is Home From Hospital?
It can be difficult to return home after being released from the hospital, especially waiting for hospital transport. Our hospital-to-home service is meant to give you peace of mind and help you recover quickly in a familiar setting.
Our Home from Hospital service will be provided by a fully trained, vetted and insured carer who will come to collect you from the hospital. This would include them supporting you to ensure you have all your belongings and a discharge form. Once arriving back home, they can stay and help settle you in. They will also be able to undertake any further home support tasks such as shopping and general cleaning if needed. If you are a new client of ours, we may need to come out to the property beforehand to assess the environment to ensure it is safe for yourself and our carers. This would be at no extra cost
